Report a deed restriction violation

This form is currently for testing only!

This form is the preferred method for submitting forms. This method will send an email to whatever addresses we specify, as well as send a copy to the person who submitted the form (if they included their email address). The message will contain all of the form fields and the submitted information as well as some hidden info that will identify which form was submitted. When a logged in person accesses a form, it will automatically fill in some of the fields for them (like Name and Email). One of the real advantages with this type of form is that it will track the form submissions. After a person has submitted a form, the next time they go to that form page they'll see a "View your previous submissions" link (this only works for logged in individuals). There, they'll be able to view all of the forms they've ever submitted. Website admins will also be able to view form submissions. On Dan's Menu, I've included a link to "Manage Webforms". Normally, access to this location will be highly restricted, but you can see the form results there. I currently have this form set up to email both Randy and Dan, so if you want to test it out, you can see what the results look like.

To report a deed restriction violation, please complete this form.

NOTE: You must include the violator's address in order for the manager to work on this matter.

A copy of the form data will be sent to this address.
